The example of the part of the dialogue:
-Have you ever had eye infections? – No, I have never had eye infections.
-Have you been troubled by running nose lately? – Yes, I have been troubled by running nose lately.
-Has your voice ever been hoarse when you didn’t have a cold? - Yes, my voice has been hoarse when I didn’t have a cold.
-Has your doctor told you that your tonsils have been enlarged? – No, he has not.
-Have you had any trouble with your eyes in the last two years? – No, I have not had any trouble with my eyes in the last two years.
-Have you been bothered by a thumping or racing heart? – I have been bothered by a racing heart.
-Have you ever been told that you may have high blood sugar? – Yes, I have been told that I may have high blood sugar.
